Start your application process by filling out this form, and then submit it. Upon submission, there will be a follow up phone interview. Acceptance is dependent on several factors, here are some examples of a few; space availability, phone interview, and application.

After the application has been approved, complete these two forms next. Click on the following links to download our required documents before your first session. Please fill out and scan signed documents and return to "cmhscoop7@gmail.com". If you do not have access to a scanner, please send us an email and we will send you a mailing address. Photos of the signed form will not be accepted.
The behavior form must be signed by a parent and by students 11 years and older. The goal of this form is to create an environment where children walk away knowing that Jesus does not support anger, bitterness, evil speaking, malice, etc (Ephesians 4:31). It may happen in our group (none of us is perfect). But if it does, it does not represent Christ and THAT is why we will address the offender and protect the "weak".
